Overview

Follow the instructions below to complete a case task, which involves surveying an exposed contact or reported person as well as identifying additional exposed contacts that need to be followed up with.

Complete a Case Task

  1. Follow the View Case Tasks guide to navigate to a list of your case tasks, and select the case task that will be worked on.

  2. Click Start Work at the top right corner of the screen. This will change the  State to Work in Progress.

  3. Verify that all fields on the Case Task are correct and filled out sufficiently based on your current information.

  4. Set the reach out date to the current date.

  5. Call the exposed or reported person, and use the call introduction script and instructions on the Details tab.

  6. Click Take Survey at the top right corner or the screen to begin surveying the affected individual.

  7. Follow the survey instructions, and pay attention to the text in blue. All blue text is instructions, and notifies you of which sections of the survey to go through with the contact on the phone. Fill in the fields that appear in each section.

  8. If you are conducting the Exposure Case Investigator Survey - UCI Campus survey for a reported person, then when you get to the Contact Overview section, you will be told to navigate back to the original case and fill in possible exposed contacts. If you are completing a case task for an exposed contact, skip to step 12.

  9. Click Save at the top of the survey to save your current information.

  10. Click Update on the Case Task record to make sure any field changes made are saved.

  11. Follow the Identify and Add Exposed Contacts to a Case guide to add all exposed contacts to the original case record, before coming back to complete the survey.
  12. Complete the rest of the survey and click Submit at the top of the survey to save the assessment.

  13. Click on the Test Details tab.

  14. If necessary, ask the contact for information so you can fill out the Have you already been tested?Has Spoken to Dept of Health Rep, and the Department of Public Health Contact Date fields.

  15. Click on the Call Closing tab.

  16. Follow the Call Closing script and instructions before getting off the phone with the contact.

  17. If a referral needs to be made, click on the Notes tab.

  18. Check the Refer to Supervisor box and enter in the relevant referral information in the Work Notes field.

  19. Select a value from the Initial Interview Outcome dropdown.

  20. Select a value from the Outcome dropdown.

  21. If the case task can be closed, meaning either all information is complete and there is no more work or follow up to be done, select a value from the Closed Reason dropdown.

  22. If the case task can be closed, click Close Task at the top right of the screen to set the state to Closed.

  23. Otherwise, click Update at the top right of the screen to save your changes and exit the record.

  24. Continue to do this for all case tasks linked to a case, and then follow the Close a Case guide to close the original case.
