Overview

After creating a new case for an affected individual, the next step is to follow up with them to update the case record with personal information and exposed contactsThis guide assumes you are currently working on a case task where the Reported Person is assigned to the Exposed Contact field. You are in the middle of surveying the contact, and you will now use the following instructions to identify and add contacts to the Exposed Contacts list, before moving on to finish the survey and case task work.

Identify and Add Exposed Contacts to a Case

  1. Follow the View Cases Assigned to Me guide to navigate to your assigned cases, and select the case record to update.


  2. Once on the case record, verify that all current field information is correct and sufficient.

  3. Click Start Work at the top right of the screen once you are ready to call the reported person. This changes the Case State from Assigned to Work in Progress.

    If the Person Details tab has not yet been filled out, scroll down and click on the tab.

    Call the reported person.
    If necessary, ask the reported person for the person details information to fill the First Name, Last Name, Sex, DOB, Age, Address, Phone, and Email fields and the Student/Faculty/Staff dropdown.

    Otherwise, click on the Click on the Creating New Contacts tab, which contains the script for adding potentially exposed contacts to the record.

  4. Work your way through the script with the reported person.

  5. For each individual the reported person exposed contact names, scroll  scroll down and click the on the Exposed Contacts tab.

  6. Click New to add the exposed person to the .

  7. Enter the contact's name in the User field.

  8. Select a value from the Type of Contact dropdown.

  9. Select a value from the Ever Symptomatic dropdown.

  10. If the exposed contact is someone that collaborated with the reported individual, check the Recently Collaborated field.

  11. Enter the reported age of the contact in the Reported Age field.

  12. Enter the Local Health Jurisdiction.

  13. Select a value from the Household Relationship field if the contact lives in the same household as the reported individual.

  14. Select a value from the Person Immunocompromised or Pregnant field.

  15. Select a value from the Health Status dropdown.

  16. Finally, click Submit at the top right corner of the screen to save the exposed contact information.

  17. The new contact will appear in the Exposed Contacts list.

  18. Continue this process for every contact that the reported person names.
  19. After finished with adding contacts, navigate back to the Complete a Case Task guide to finish the survey questions and complete task work.
  20. Once finished surveying, make sure to follow the Create a Case Task guide to make a case task for each person in the Exposed Contacts list.
